SAMPLE WRITTEN STUDENT LEARNING PLAN

Enter ONLY the off-site course work (at home, lessons outside of KCHS, etc)
Only enter subjects your student will be studying. Do not enter Workshops taken at KCHS, those will prefill. Written Learning Plans must meet the following minimum criteria:
• Include beginning and end date. (this will prefill)
• Estimate of weekly hours will prefill based on grade level
• Specify how weekly contact requirements will be met. (You must select one or all types)
• Include learning Description and materials, Goals & Activities and Method of Evaluation.
• Be developed and monitored by certificated staff.
• Available: connection with Essential Academic Learning.

Beginning Date: Begin-End Date(this prefills-you don't have to add it)

Art
Description: Self-directed art and craft projects at home. Piano lessons and daily practice. Will visit art galleries and museums.

Goal: To appreciate and participate in the arts to enrich his/her life.

Assessment: Parent critiquing of work and completion of projects.

Computer Education
Description: Using the computer and word processing programs for all writing projects, Editing program and selected math activities.

Goal: To be comfortable with computers and their uses as tools to help in his/her work.

Assessment: Parent observation and review.

Health/PE
Description: Study nutrition, human body and safety skills including first aid. Walks with family, walking family dog.

Goals & Activities: To establish a healthy lifestyle.
Learn about cooperative games including volleyball, soccer, baseball, and basketball. Learn about physical conditioning, jump rope, tumbling, juggling and dance class. Evaluating food choices by discussion and helping with meal planning, food purchases and meal preparation.

Assessment: By parent through discussion with student and observation.

Language Arts
Description: To read for pleasure, keeping a reading log. Reading with a literature based history curriculum. Working with [ ] curriculum on the computer at home. Focusing on the 6 Traits of Writing with emphasis on voice, word choice, organization and the use of grammar, spelling and punctuation Writing letters to friends and family.


Goals & Activities: Goal: To challenge student in both reading and writing and show progress in writing and editing skills.

Assessment: Parent observation, quizzes and oral self-evaluation of progress.

Mathematics
Description: Student will have continued practice in solving problems by using a guide, choosing an operation, guessing and checking, looking for patterns, drawing a picture, and using logical reasoning.

The following resources will be used to assist in understanding and learning these concepts?
Scott Foresman Mathematics, level 5, Chapter one; various math-related web sites and computer software

Goals & Activities: Student will learn:
how to use line graphs and stem and leaf plots to analyze data how to find the range, mode, and median for a set of data how to choose a scale when making a bar graph.

Assessment: Through daily assignments and weekly tests.to reinforce basic skills. Create and compute decimals and percents. Add, subtract, multiply and divide. Learn use of fractions using text and workbooks. Weekly review with parent.

Occupational Education
Description: Managing schoolwork, homework and chores at home. Keeping own schedule and being responsible for own time.

Goal: To become self-motivated and a better time manager.

Assessment: Parent observation

Science
Description: Study the physical and environmental science including matter, variables and solar energy, learn about the solar system or ocean habitats study trees and forests during an outdoor experience. Daily readings and activities and weekly tests.

Goal: To complete general science curriculum.

Assessment: Graded tests. Parent observation of progress.

Social Studies
Description: Reading biographies and historical fiction. Watching a video course on Early American History. Visiting various museums and participating in cultural outings. Study the early civilizations of the Western Hemisphere including ancient Egypt, Greece and Rome, develop an understanding of the Middle Ages with emphasis on the Renaissance, research modern civilizations including an in-depth study of a country, discuss the effects of Asian cultures on the Western Hemisphere

Goal: To become more familiar with events in American History with a focus on Civil War to present.

Assessment: Discussion of books read and video course with parents and other family members and keeping a reading log.

World Languages
Description: Attending “silent games night” and other outside events to continue ASL skills. Using a tutor, complete Latin I.

Goal: To be able to communicate with and understand other cultures.

Assessment: Parent evaluation of class work outside activities and observation.
