9/16/16: Final Clearance Letter from Snohomish County Health

Dear Mr. Mannix,

The Snohomish Health District has reviewed your letter of 9/9/16 and the associated documents. Additionally, we are aware you have provided EPA with the materials and information they requested as outlined in our letter of September 15, 2016.

To this end, the Snohomish Health District finds you are in conformance with the Corrective Action Plan dated May 25, 2016. Consequently, Snohomish Health District has no objection to use of the school facility.
Additionally, use of the facility is conditioned upon conformance with your plan to take a set of four (4) air samples - one per quarter - in all spaces where PCB caulk was removed from the interior around windows, doors, or structural columns (as specified in item #4 of your plan). In the unexpected event the School District fails to conform to this portion of the corrective action plan or any EPA stipulated requirements, and/or test results indicate conditions that a health risk may exist, Snohomish Health District may alter our determination that the school can operate.

I thank you for your continued cooperation throughout this situation and appreciate the School District's ongoing commitment to student health and safety.

Dear Mr. Mannix,

The Snohomish Health District has reviewed your letter and the associated documents of 9/9/2016 and agrees that based on the information provided, conformance with the Corrective Action Plan has been achieved.
Consequently, SHD has no objection to use of the facility next week for the purposes you describe (orientation, planning, etc.).
In response to your report, EPA has raised seven issues you wlll need to adequately address:

1. Final QA/QC data for the wipe samples of small gym floor and exterior of Annex Building.
2. Area (in square feet) of small gym floor and area exterior of Annex building that were re-cleaned.
3. Rational for collecting one sample of the floor in the small gym.
4. Please report all air sample quantitation limits/reporting limits as ng/m3
5. Were the wipe samples collected after caulk removal collected before or after epoxy encapsulation?
6. Explain the filter analysis procedure used for air samples run under Method TO-lOA.
7. Report whether the air sam pies were collected under normal building HVAC operating conditions?

SHD expects that you will address these issues no later than 9/12/16. In the unexpected event your responses cause EPA to question the results or determine that a health risk may exist, Snohomish Health District may alter our determination that the school can operate.

Additionally, as there are expectations of ongoing monitoring and associated action plans that need to be addressed, the Snohomish Health District will be preparing a letter summar izing these conditions and issues. This letter will be sent to you no later than Thursday 9/16/16.
